What are the actual causes of chronic Prostatis, itching in the genital area and urine retention? How to permanently prevent it? Can antibiotics (like levofloxin)/flowmax (for voiding) help?

Not necessarily. Prostatitis can be caused by any organism which can make its way into the prostate. This includes bacteria and viruses as well as yeast and fungus. A proper diagnosis is make by culture of prostate fluid. Urologists are the people who take these types of cultures. Also, prostatitis can be caused by autoimmunity and this may be a trigger for prostate cancer. I suggest that anyone with prostatitis put themselves in the care of a competent urologist.

The usual ways of entry into the body by organisms is through the mouth, through the lungs, via breaks in skin integrity; they are transported by blood and lymph flow. In addition, the prostate can be infected though the urethra. As to prevention, like any other threat of infection, the best protection is a strong immune system. This is not a pill you pop, but an entire lifestyle.
